Salary: Band: 10, £41,510 - £45,718 p.a - + shift allowance of 7.5% if applicable

The Service
The Community Rehab Service (CRS) is an integrated health and social care service designed to support adults with health and care needs to continue living in their own homes as independently as possible for as long as possible.
We are ambitious in our plans to deliver innovative and effective services that put the customer at the centre of what we do, working in more flexible ways to understand how we can put the right support in the right place at the right time to help people live the lives they want to live. By doing this, our aim is to improve the quality of care provided, reducing demand for social care services, reducing acute admissions (and therefore increased demand for ASC services) and length of time spent in hospital, as well as reducing the dependency on long term care.
The role
This is an exciting opportunity for an Occupational Therapist where you will deliver reablement and rehabilitation interventions to Portsmouth residents in their own homes to restore, maximise or prevent deterioration in their physical, psychological and social functioning. The role will involve you assessing the rehab and reablement needs of clients referred to the service (including those with complex presentation), planning treatment interventions and determining and evaluating agreed outcomes. You will monitor, evaluate and modify interventions to measure client progress and treatment effectiveness evaluating outcomes and arranging transfers of care if appropriate. You will be responsible for completing risk assessments and provide spontaneous and planned advice, teaching and instruction to clients, carers, relatives and other professionals. In addition, you will support and educate rehabilitation assistants as well as supervision to unqualified/qualified staff.
What you'll need to succeed
You will have a Diploma of the College of Occupational Therapists or BSc Occupational Therapy or OT qualification, substantial post qualifying experience and a current registration with HCPC. With a thorough understanding of the principles of rehabilitation, reablement and enablement you will have the ability to formulate, modify and evaluate rehabilitation programmes. You will have experience of complex caseload management and providing specialist equipment and using assistive technologies to improve, restore and attain occupational performance. With knowledge of relevant legislation relating to adults with disabilities you will have experience of manual handling and formulating functional assessments of complex needs and devising action plans.
With excellent communication skills you will have experience of working under pressure t meet deadlines whilst responding to competing demands and changing priorities. You will be fully conversant with the eligibility criteria for Adult Social Care.
